School Administration of Medications Guidelines

When should a student or teacher stay home from school?

If your child needs prescription medication while at school, this form provides the authorization our nurses need to administer it safely. Please submit the completed form and medication to the school nurse.

This form allows parents and guardians to authorize school staff to provide approved over-the-counter medications during the school day. Please return the completed form to the school nurse.

This form allows students with asthma to carry and self-administer their prescribed inhaler during the school day. Please submit the completed form to the school nurse.

This form allows families to permit a student to carry and self-administer their EpiPen while at school. Please return the completed form to the school nurse.

These care plans are available to help families and school staff prepare for and respond to students’ specific health needs during the school day. If your child has a diagnosed allergy, asthma, or seizure disorder, please complete the appropriate care plan below and return it to your child’s school nurse.
